How to fill up the Annapurna Bhandar form (section by section)

The exact layout of the form can change. The sections below are what applicants usually need to fill in; follow the headings printed on your form.

1. Applicant’s personal details

  • Name: write exactly as in your Aadhaar card, in CAPITAL letters.
  • Father’s / husband’s name: as in your documents.
  • Date of birth: in the format asked (usually DD/MM/YYYY). It must match Aadhaar. Your age decides eligibility. Age limit: To be confirmed, check the official notice.
  • Gender, category (SC/ST/OBC/General), marital status: tick the correct box.

2. Address details

Write your full permanent address in West Bengal: house number, village/ward, post office, police station, block/municipality, district and PIN code. It should match your residence proof.

3. Contact details

Give a mobile number you will keep for a long time. All OTPs and payment SMS will come to this number.

4. Identity details

Write your Aadhaar number carefully and check every digit twice. Fill in other ID numbers asked for (such as Swasthya Sathi or ration card) if you have them.

5. Bank account details

  • Account holder name: must be the applicant herself.
  • Account number: copy from the passbook, digit by digit.
  • IFSC code: copy from the passbook first page; it has 11 characters, and the 5th character is always zero (0).
  • Bank and branch name.

6. Declaration, photo and signature

  • Paste one recent passport-size colour photo in the box. Do not staple through the face.
  • Sign or give your thumb impression in the declaration box. Use the same signature you use at your bank.
  • Write the date and place.

  • Using small letters or cursive handwriting that is hard to read
  • Overwriting or using whitener: ask for a fresh form instead
  • Name spelling different from Aadhaar (e.g. “Rina” vs “Reena”)
  • Wrong or missing IFSC code
  • Bank account in husband’s or son’s name
  • Photo not pasted, or signature missing
  • Leaving mandatory fields blank
  • Submitting the form twice

After submitting the form
